What are chronic diseases?
Diseases such as cardiovascular disease, diabetes, COPD, asthma, kidney disease and others are categorised as chronic diseases. These on-going or long-standing diseases can be debilitating and have dramatic effects but, in many cases, can be managed well so that you can have the best possible quality of life.
If you have a chronic disease, you may be able to get Medicare Benefits to see allied health professionals (physiotherapist, psychologist, podiatrist, etc) to help manage that condition.
Our GPs and nurses can determine whether you are eligible for these services and draw up a Chronic Disease Management Plan to help you access services needed and continue to monitor your progress.
